High Performance Team Questionnaire

For the team you lead or the team of which you are a member please respond to each of the following questions by placing a number from 1 to 5 (1 = small extent; 5 = to a great extent) that you believe describes the situation in the team. Then score your results.
1. Do team members communicate easily and frequently?
2. Is the team the right size to perform effectively?
3. Are problem-solving and inter-personal skills represented across the team?
4. Is there a team purpose as opposed to a broader organisational purpose or just one individual’s purpose??
5. Are there team goals as opposed to broader organisational goals or individual goals?
6. Do the team’s goals require collective as opposed to individual outputs?
7. Do members share common agreement about the the team’s goals?
8. Is there a clear working approach understood and agreed by everyone?
9. Are team members collectively accountable for the team’s output?
10. Are team members clear on what they are collectively responsible for?
